Clinical
The dimensions your clinicians score, carried into the plan they write.
ASAM assessments are one of the 24 standardized instruments in the platform. The six dimensions are scored on your own form, and the treatment plan that follows is built from them — so the placement decision and the plan that acts on it are the same record.
Your clinicians decide the level. Unisona Health records the assessment and the plan; it does not compute a level of care for you.
The assessment
Six dimensions, scored on your own form.
The criteria are a clinical framework, not a software feature, so the form follows the edition and the wording your program uses. Each dimension is rated on the 0–4 severity scale and saved as structured data on the chart, not as a paragraph someone has to read twice. The assessment is one of the 24 standardized instruments in the platform.
Acute risk, and what is being used to manage it.
Medical problems that shape where care can safely happen.
Co-occurring conditions and current stability.
Where the patient actually is, not where the program wants them.
Risk of return to use without this level of support.
Housing, relationships and the setting the patient goes back to.
From assessment to plan
The plan is written from the assessment, then tracked.
A placement decision that never reaches the plan is a decision nobody acts on. Here the completed ASAM assessment is what the treatment plan is built from, and the plan is tracked from then on.
Written from it
Objectives tied to dimensions
Goals, measurable objectives and interventions each carry the ASAM dimension they answer, a cadence, a status and a target date.
Then tracked
Plan completion is visible
Once the assessment is done and the plan is written, its progress and reviews are tracked on the record rather than remembered by the clinician who wrote it.
What this does not do
Unisona Health records the level of care your clinician determined. It does not compute one from the scores.
Placement is a clinical judgement, and a platform that guesses at it produces a number somebody then has to defend. What the record shows is what was assessed, what was decided, and who decided it — which is what a reviewer asks for.
Questions
Common questions.
Does Unisona Health support the ASAM Criteria?
Yes. ASAM assessments are one of the 24 standardized instruments in the platform. The six dimensions are scored on the assessment form, each rated 0–4, and the scores are recorded on the chart alongside the rest of the clinical record.
Does it decide the level of care for us?
No, and deliberately so. The platform records the dimension scores and the level of care your clinician determined; it does not calculate or recommend a placement. The clinical judgement stays with the clinician, and what the record shows is what they assessed and decided.
How does an ASAM assessment connect to the treatment plan?
The plan is written from the completed assessment. Goals and measurable objectives are tied to ASAM dimensions, each with a cadence, a status and a target date, so the plan acts on the dimensions the assessment flagged rather than repeating them as prose. Progress against the plan is then tracked on the record.
Which edition of the ASAM Criteria does the form follow?
Whichever one your program uses. Assessment forms are built in the form builder, so the dimension headings, required fields and scoring follow your own version, and the form can be revised without waiting on a vendor release.
Can we produce ASAM level-of-care reviews for an audit?
Yes. Completed assessments and level-of-care reviews sit on the patient record and are pulled into an audit response packet with the rest of the chart — paginated, in date order. Because these are SUD records, 42 CFR Part 2 consent is enforced on every export path.
