Clinical

Build the note your accreditor already expects.

Most systems give you their note types and wish you luck. Unisona Health lets you build your own — field by field — then locks the version so every clinician charts the same way.

See what it costs

No developer required. Clinical leadership builds and publishes the form.

The builder

What you drag on the left is what a payer reads on the right.

Clinical leadership assembles the note from field types — no ticket, no vendor, no release cycle. The published form is what every clinician sees the next time they open that note type.

Versioning

Change the form without rewriting history.

When you publish a new version, notes signed on the old one keep rendering exactly as they were signed. An auditor reading a note from two years ago sees the form that was in force that day.

Accreditation

Chart the way you're surveyed, not the way a vendor guessed.

CARF and Joint Commission reviewers look for specific elements in specific note types. Putting them in the form is more reliable than putting them in a training memo and hoping.

Questions

Common questions.

Can I build my own note types and forms?

Yes. Clinical leadership assembles a note field by field from field types — long and short text, single and multi-select, scored scales, checklists, signatures and co-signatures, conditional sections, carry-forward fields and more — then publishes it. No ticket, no vendor, no release cycle, and no developer required.

What happens to old notes when a form changes?

They stay exactly as they were signed. When you publish a new version, notes signed on the old one keep rendering under that version, so an auditor reading a note from two years ago sees the form that was in force that day.

Can forms enforce what accreditors require?

Yes. CARF and Joint Commission reviewers look for specific elements in specific note types, and putting them in the form — as required fields, ASAM scoring, or a required supervisor co-signature — is more reliable than putting them in a training memo and hoping.
